There are many times when a patient is unable to answer the questions of a health history. Whether it be they are disabled, obtunded, pediatric or cognitively unable to. Each case is different in the way that I would collect information. With pediatric patients, I would look to the parents or guardians for the answers to the questions. In a geriatric patient, sometimes just taking the time to allow them to answer or collecting information over a series of visits will establish a report and trust of the patient and the collection of information can be done. When it comes to a patient that is obtunded and does not have family or anyone that can offer information, contacting their physician, if able is one way to collect information in an emergent situation. Another would be to focus on the care that is needed and continue to try to reach out to family or friends that may be able to give any type of information that would be helpful. These strategies are effective because it is a provider’s duty to create trust and comfort to a patient and the ability to learn to collect information in not so perfect situations is imperative. Family, interpreters, EMR if it is linked to the patient’s physicians are all ways to collect. It is important to be able to get some information in order to give quality care and positive outcomes.
