Please download and use this attached template to make sure your Annotated Bibliography is in proper APA format. Once opened, please carefully edit the header on the Title Page and then again on Page 2.  You must change the words SHORTENED TITLE on both pages to a short version of your document’s full title.  

For Example:

Full Title: Early Onset of Obsessive Compulsive Disorder

SHORTENED TITLE: EARLY ONSET OF OCD

Please take note that the SHORTENED TITLE must be in ALL CAPS!
